Suggested Materials for your Follow Up Oncologist Meeting
Keep a running log of the following key measurements so as to spot possible trends: BP, O2 Saturation, Creatinine, Weight, Thyroid values etc
Keep a copy of the latest blood work and scan interpretation in case you need to reference it during your appointment with your Oncologist.
Include your Journal to take notes (reference details below)
Medical Documentation for your Personal Files: (Binder at Home)
Make copies of all of your scans, get a copy of the scan itself on a CD from the hospital’s Medical Library and keep a hard copy of the radiologist’s interpretation of the scan. Your other doctors GP, Pulmonologist, Cardiologist etc may need to review, make a copy of the report findings
Lab results printout especially if you have some values that are outside the normal range. Track various blood measured values over time to check for trends ie creatinine etc
Keep a Journal in your Bag
Take observation notes periodically and make a list of questions or symptoms.
Write down your questions before your Oncologist meeting and leave room to write in the answers below each question in your discussion notes.
Track your blood work values, BP or those measures ie creatinine over time if they are out of normal range. Capture them in the dated log for that appointment.
Capture information on topics you want to research such as information on promising clinical trials etc
Create a log by date for each doctors appointment or treatment appointment. It can be easier to refer back to information captured in one place by date vs shuffling printouts if you arent technically inclined.
